May 30, 2003 – Application forms for the 2002 Ontario Sport Awards are now being accepted by the Sport Alliance of Ontario. Nominations can be made in the following categories:

– Ontario Male and Female Athlete of the Year
– Ontario Male and Female Disabled Athlete of the Year
– Ontario Male and Female Coach of the Year
– Ontario Team of the Year
– Corporate Citation Sport Citation
– Special Achievement Award

Applications are available in two formats from the Sport Alliance website:

In an effort to ensure accuracy, legibility and the ability to import and manipulate the data, the Ontario Sport Awards Selection Committee requests that applications be completed, if possible, via a word-processor [preference] or typewriter.
